Arranging a service

Start with the place, the people and the work.

For workplace services, tell us where the site is, approximately how many people need to be seen and what their work involves. If you already have a risk assessment, send it to us. If you do not, we can help establish what is required.

For individual care, contact the practice with a short description of what you need. Please do not include private medical details in the website form; we will discuss those with you through an appropriate channel.
